Enteral Nutrition Forumlary Changes

The Enteral Nutrition Formulary will be changing, which was approved by the Pharmacy and Therapeutics committee at the October 1, 2013 meeting. Please see the attachment for a list of previous formulary products and the new formulary product names. We will "go live" with use of these tube feeding products and oral nutrition supplements on Tuesday, November 5. This change will not affect the quality of nutrition to our patients. The products are the same – just the names are changing. This change in products is necessitated by a Med Assets contract change from Abbott to Nestle Health Science products.

Pharmacy will have these formulary changes in Meditech and the CIS Team will be updating the order sets and physician's favorites by the "go-live" date of November 5. During the first couple of weeks of this conversion, you may see some of the previous formulary products still being used until we have used up our current stock. Therefore, you may order Fibersource HN, but your patient may receive Jevity 1.2 Cal until we have exhausted our current stock.
